1. Acceptance of Terms
By accessing or using the Trustora platform, you agree to be bound by these Terms and Conditions. If you do not agree, do not use the platform. Trustora is intended for authorized healthcare professionals and agency staff only.
2. Account Responsibilities
You are responsible for maintaining the confidentiality of your account credentials. You must not share your password, OTP codes, or signing passcode with anyone. You must immediately report any unauthorized access to your agency administrator. All activity under your account is your responsibility.
3. Acceptable Use
You agree to use Trustora only for authorized agency work. You must not: access client records outside your assigned caseload; share PHI outside approved workflows; attempt to circumvent access controls or audit logging; use the platform for any unlawful purpose; or export bulk data without authorization.
4. Protected Health Information
All clinical data entered into Trustora is Protected Health Information (PHI) under HIPAA. You must handle PHI in accordance with your agency's HIPAA policies, the minimum necessary standard, and all applicable federal and state regulations. Unauthorized disclosure of PHI may result in civil and criminal penalties.
5. Clinical Documentation Standards
All clinical documentation (Functional Assessments, Treatment Plans, Progress Notes, Diagnostic Assessments) must meet Minnesota DHS ARMHS documentation standards. Digital signatures are legally binding. You are responsible for the accuracy and timeliness of all documentation submitted under your credentials.
6. Audit and Monitoring
All activity on Trustora is logged and monitored for security, compliance, and audit purposes. Audit logs are immutable and retained for a minimum of 7 years. Your agency administrator and authorized compliance officers may review your activity at any time.
7. Service Availability
Trustora strives for 99.9% uptime but does not guarantee uninterrupted service. Scheduled maintenance will be communicated in advance. We are not liable for any damages resulting from service interruptions.
8. Termination
Your access may be terminated by your agency administrator at any time. Trustora reserves the right to suspend accounts involved in unauthorized activity. Upon termination, your access to clinical records ceases immediately, but audit records are retained per compliance requirements.
9. Account Deletion
You may request deletion of your account at any time. See our Account Deletion page for details. Note that clinical records and audit logs cannot be deleted due to regulatory requirements.
